Life insurance

To submit your Life or Accidental Death claim for fast-track processing,
- Print, complete and sign this claim form
- Attach a copy of the Death Certificate
- Attach a copy of the will ( if one exists )
- Send to Asteron by:

We may require Probate or Letters of Administration particularly if the claim value is greater than $15,000.00 (if this is required we will contact you).
We endeavour to process your claim as quickly and seamlessly as possible, however there may be occasions where other documentation is required – we’ll let you know if this is the case.
